What is Home Care?
Home care encompasses a vast array of medical and social services. The convalescent, disabled, chronically ill, or terminally ill individuals who require medical, nursing, physical therapy, social care, or assistance with activities of daily life receive these services at home.
When a person needs ongoing care that cannot be easily or efficiently supplied by family and friends alone but does not require hospitalization, home care is appropriate. As their physical capacities decline, an increasing number of older persons who choose to live independent, non-institutional lifestyles are obtaining home care services. Increasing numbers of patients require highly specialized care when they return home as hospital stays decline. Some patients can receive safe and efficient care while remaining in the comfort of their own homes.
What is "home bound" status?
According to the legislation, a doctor must always certify that a patient is restricted to his or her house for Medicare to pay for home health services. For the purposes of the law, a person is deemed "limited to the home" (home bound") if both of the following two conditions are satisfied:
Criteria 1: Need assistance leaving their place of residence due to disease or injury, such as using crutches, canes, wheelchairs, or walkers, special transportation, using a vehicle, or having someone else help them.
Criteria 2: A typical inability to leave home must exist and It must be difficult and exhausting to leave home.
Even if the patient does leave the house, they may still be seen as home-bound if they only leave the house occasionally or for very brief periods of time. These specifications don't apply to:
participation in a state-licensed adult day care center participation in a religious ceremony absenteeism due to illness.
Home-bound definition includes:
Absences are few, brief, and do not suggest that the patient has the ability to receive medical treatment delivered outside of the house.
If a patient's condition, brought on by a sickness or accident, prevents them from leaving their home without the help of a cane, wheelchair, walker, or the most amount of assistance from another person.
if a patient's condition makes leaving home contraindicated by their health.
Any absence from religious services is sporadic or brief, and as a result, does not disprove home-bound status.
Can I choose a home health care provider?
According to the Law, Home health care patients are permitted by federal law to select the organization or individual who will deliver their in-home care services. "Seeks to ensure that free choice is granted to all Medicare Patients," says Section 1802 of the Social Security Act. Any person eligible for insurance benefits under this title (i.e., Medicare) may acquire health services from any institution, agency, or person qualified to participate under this title if such institution, agency, or person commits to provide him with such services, according to the law.
How can home care services be initiated?
A doctor's prescription can be used to start home care services. A discharge planner will help with the referral process before discharge if a patient is in the hospital. Our nurse will arrange a visit to do an initial evaluation once we receive the order. The nurse will inform the doctor of her findings and offer recommendations for treatment.
Does a patient have to be hospitalized in order to be entitled to home care?
To be eligible for home care, patients do not need to be admitted to the hospital. After a clinic visit or anytime, he decides there is a need for services, your doctor may make a referral.
Who will pay for the services?
Medicare, Medicaid, Blue Cross Blue Shield, and other private insurances all provide coverage for services. We will check for coverage with Medicare or a private insurance provider before offering services.
We do accept private pay as well.
Will personal information be kept private?
All contracted service providers, including Freedom Caregiver Services, LLC adhere to HIPPA's (Health Information Privacy and Portability Act) regulations. Only employees who are directly involved in providing care have access to personal and medical information.
How frequent are the visits and for how long?
Services are covered by Medicare, Medicaid, Blue Cross blue Shield, and other private insurances. Prior to providing services, we will verify coverage with Medicare or a private insurance carrier. We do accept private pay as well.
Are family members involved?
Patients and their families are required to engage in their care because home health care is a transient sort of service. Our staff members are here to inform, instruct, and support patients in taking care of their condition, receiving treatment, and performing other tasks that must be done at home.
If a patient is admitted to the hospital, will services resume after discharge?
We will stay in touch with the patient's doctor and family members while they are in the hospital. In order to continue with home-care after discharge, we will ask the hospital for discharge information.
Are medical supplies and durable equipment’s covered?
Medicare and other insurances cover most supplies and durable medical equipment. If some expenses are not reimbursed, we will inform the patient.
What happens if a patient gets better?
Our personnel will continuously assess their condition. They will approach the doctor for permission to dismiss a patient from the program if they decide that they are healthy enough to take care of themselves.
Will physicians be involved?
A physician will supervise home care and be informed of a patient's development. They will issue all prescriptions for medical care.
Will care providers call for appointments?
Freedom LLC provides Home Care Services. When you have an appointment, the healthcare practitioners will call you at least one day before to confirm. If you have any other appointments, please let them know so they can adjust their schedule accordingly.
Can services be cancelled?
Patients have the freedom to choose how they are treated, as well as their family members. They are free to discontinue services at any time and for any cause.
